Please click on the link below to find the 4-H health forms. 4-H'ers that want to attend 4-H camps need to fill out the Louisiana 4-H Medical/Health Form, the Louisiana 4-H Youth Code of Conduct Form, and the Louisiana 4-H Hold Harmless Agreement Form.
The Louisiana 4-H Certification of Medical/Health Form Being Current and Correct is to be used after the Medical/Health Form has been completed and submitted. This form affirms all information on the already submitted health form is still correct and current. In the case of any information changing, a new Medical/Health Form must be resubmitted.
The Louisiana 4-H Hold Harmless Agreement Form is part of the youth and adult enrollment forms and are only required if the form was not completed during the enrollment process.
Find the 4-H health forms here. All of the forms are fillable online. Each 4-H'er must fill out, sign, and submit all forms needed prior to leaving for camp. The completed forms can be faxed to the Ouachita 4-H office (318-387-9376), mailed via the United States Postal Service (704 Cypress Street, West Monroe, LA 71291), or dropped off at our back door in our Lock Box.
