Two life insurance plans are currently offered to LSU AgCenter employees.
Prudential (through the Office of Group Benefits)
LSU Supplemental Life(UnitedHealthcare)
PRUDENTIAL (Office of Group Benefits) - OGB offers two fully-insured life insurance plans for employees and retirees through The Prudential Insurance Company of America (Prudential). LSU pays half of the life insurance premium for covered employees and retirees. Coverage is guaranteed if you enroll within 30 days of your benefits eligibility. If you do not enroll as a New Hire, you and your spouse can enroll anytime with Evidence of Insurability. Dependent child(ren) do not require Evidence of Insurability. LSU SYSTEM TERM LIFE INSURANCE (Administered by UnitedHealthcare) - This plan provides an option for group-term life coverage for eligible employees. It provides a death benefit equal to the coverage amount in effect at the time of death and payable to the named beneficiary. Coverage amounts available range from $10,000 to $1,000,000. Premiums are age-rated. Also available is accidental death and dismemberment coverage.
Dependent life insurance can be purchased for the employee's spouse and children. Coverage for children may be added anytime without proving Evidence of Insurability.
Eligibility for Enrollment
Employees appointed for more than 120 days and 75% of full-time or greater are eligible for enrollment.
Effective Date of Coverage
Within 30 days of new hire, the plan guarantees issue of coverage up to 3 times your annual salary (in $10,000 increments) or $350,000, whichever is less. Premiums will adjust every January 1, according to age.
Spouse coverage is available up to 50% of the amount of employee coverage, but only up to $100,000 when enrolled within the first 30 days of new hire or within 30 days of a family qualifying event.
Children may also be covered with no medical information required. Amounts are $5,000, $10,000, $15,000 and $20,000 for each eligible dependent child (14 days - 26 years). AD&D coverage also available to match life amount, not to exceed 50% of employee's benefit amount. It is the employee's responsibility to notify the Human Resource office to stop deductions once a child is no longer eligible.
