Documentation Required For Sampling
Chain of Custody Form

The Chain of Custody form is required to accompany regulatory samples from EPA- and LDEQ-funded projects. All other samples must be accompanied by a Sample Analysis Request Form. Please review the Water Sample Management and Collection Guide in the "Additional Information" link for necessary sampling and shipping guidance. If you have further questions, contact the Callegari Environmental Center Laboratory.
