- Records show your 4-H club work progress. Keep this book from year to year and add to it. You may include up to three years’ of 4-H work.
- This form is designed to help you keep your records and other supporting materials in an orderly manner.
- Do not alter the basic format of this book.
- Don’t forget to include your 4-H project book(s).
Don't wait too long to begin! The completed 4-H Record Book (Portfolio) is due to your 4-H school sponsor by their annoucement deadline, or by your April 4-H club meeting, or by April 15th at the Rapides Parish 4-H Office.
