All 4-H members who attend overnight events must submit this permission slip to their parish 4-H agent.

Standard of Excellence Award
The Natchitoches Parish Standard of Excellence Award is given to those deserving 4-H’ers who participate at an extraordinary level. This participation includes things on a club, parish, regional and state-wide level. These 4-H’ers will be recognized during the awards banquet in May. NSU Basketball Night with 4-H
January 31, 2008 is 4-H night at the NSU Demons basketball game.

4-H Code of Conduct Acceptance Form
4-H’ers are under the supervision of all Extension personnel and other adults helping with the event. Each club member will be expected to participate fully in all programs and uphold exemplary standards of behavior.
