The Code of Conduct Form should be reviewed and signed by the 4-H'er and Parent or Guardian then returned to the 4-H Office prior to the event. |
Parental Consent to Administer Medication at 4-H Camp
This form must be completed if medication is to be given to your child during a 4-H event/activity. Medication should be in original pharmacy container and, along with this form, must be given to the 4-H agent or other staff responsible for the event. |
