TO: 2009 4-H Concordia Scholarship Applicants
RE: 4-H Scholarship Forms
The 4-H Scholarship form is included for you to complete. Please mail or bring the completed form to the 4-H Office by March 2, 2009. (Office mailing address is 405 Carter Street, Vidalia, LA 71373.) Please call our office at 336-7084 if you have any questions.
4-H Scholarship Information:
Applicants for scholarships must be a 2009 graduating high school senior and entering college or technical school in the fall, 2009.
General scholarship requirements are:
1. 4-H Scholarship Application Form (extra pages added only where option is given). Application must be typed.
2. Transcript of grades; including mid-term grades signed by an appropriate school official.
3. Copy of ACT scores.
4. Letter of recommendation from an academic advisor (teacher, guidance counselor, principal) on school letterhead stationery.
5. Must meet requirements as stipulated in criteria. EXAMPLE - The Jason McClaran Scholarship requires applicants to major in dairy production or dairy foods technology at LSUBR.
5. Only the last 3 years of 4-H work is allowed.
6. All applicants will automatically be considered for the parish Scott Foundation Scholarship.
Club members submitting applications for more than one scholarship need only submit a single form.
