Employment Packets

Welcome to the LSU Agricultural Center!

As a new employee of the LSU AgCenter, there is an online packet of information with several forms that you will need to complete.  These forms will need to be completed and submitted in order for you to start receiving paychecks.  All of the forms and most of the information you need can be found on this website. 

There is a lot of information to read and a many decisions to make.  It is beneficial to discuss your options with a staff member of the Human Resource office.  There is a weekly Benefits Info Session held on Wednesdays, generally from 9 - 11am. In-office, online and phone conferences are available.  Once you have reviewed the packet, please sign up for one of the Wednesday Info Sessions. You may do so by e-mailing Kristen Manes or Dolores Nehlig.  This will help to complete the proper forms.  (Human Resource Office: 225.578.2258)

You should allow ample time to review and complete these materials and ample time if you need to contact plan representatives.  We believe this is a good investment of your time when you consider that some of the decisions you are being asked to make may affect you for years to come. 
  • Begin with Folder 1, General Employment Forms (see list of folder links below).
  • Next, read the Schedule of Employee Benefits.  This booklet provides an overview of benefits, particularly those in the retirement and insurance areas (This booklet is current for 2014 benefits. It is being updated for those available in 2015).  
    1. We offer a one-on-one session with our Benefits office.  Please call AgCenter HRM office at (225) 578 - 2258 for an appointment.  Since the orientation session will provide you with information you'll use to make major decisions regarding your retirement and insurance benefits, please feel free to bring your spouse or person you will be including in your decisions.
    2. We offer a new employee orientation program generally held on the 2nd Tuesday of scheduled months.  You must register to attend.  Here is a link that provides forms and instructions for registration.  At this orientation, our benefits personnel explain the details in a small group setting.  There are other portions of the orientation involving organizational issues and computer training, which may be mandatory for you.
  • Move to folder 2 and Folder 3 (see below) to make decisions on the major benefits (retirement and insurance) and note the following:
    1. Your retirement decision must be made within a certain time period, which is determined by the length of appointment, from the beginning of your employment.  During this period you will be receiving your salary check. (See folder #2 below for more information.)
    2. Insurance decisions must be made immediately in order to be paid.  However, changes to your coverage may be made within the first 30 days of employment.  After 30 days, changes can generally be made only during the annual enrollment period - each October unless there is a qualifying event.  It is recommended that new employees attend one of the Wednesday Benefits Info Sessions mentioned above. 
  • Move to folder 4 (see below) and use the supplemental insurances to fill other areas of coverage you may desire.
  • All non-U.S. citizens must complete the forms in Folder 5 (see below).
This material is divided into five folders.  

All employees must complete Folder 1.

  • Field Units that load directly into HRM: After loading, send Print Screen with brief job description and authorized signature along with required documents.
  • Units that do not load: You must provide the following information:
    • Name of Employee
    • Starting Date
    • Projected End Date
    • Brief Job Description
    • Account Number to be Charged
    • Rate of Pay
  • New Employee Orientation Checklist - At a minimum, complete List 1.  Retain in unit.
  • Intention to Employ a Minor - Complete if necessary.  Forward to HRM.
