Employment Packet - Folder #1 - General Employment Forms

After you have submitted the Request for Employment Packet form to the HRM Office and received the email stating which forms are required, print out the required forms for each folder and provide to your new employee for completion.

  2. Supplemental Employment Form

  3. Tax Forms (2):  W-4 Federal and L-4 State

  4. Form I-9 - Employment Eligibility Verification 

  5. Bank Deposit Authorization Form (Electronic Direct Deposit)

  6. Drug Policy Statement (AgCenter PS-18) 

  7. Bank of America Corporate Card - (For Academic & Non-classified Employees Only)

  8. Car Insurance Agreement - (For LCES  Academic Employees Only)

  9. Name Tag Information - (For Academic & Non-classified Employees Only)

  10. Selective Service Certification - for all male U.S. citizens and male resident aliens living in the U.S. who are ages 18 through 25.

  11. Civil Service Application - SF-10 - (For Classified Employees Only)

  12. Vehicle Authorization
