UNUM Long Term Care Insurance plan provides benefits for the type of care received when someone needs assistance (either at home or in a facility such as a nursing home) with the activities of daily living due to an accident, illness or advancing age.
Types of Coverage Available and Rates
Monthly benefit amounts available range from $1,000 to $4,000. The benefit duration is either 3 years or 6 years. Premiums are based upon the age at time of enrollment. See below for the rate information.
Eligibility for Enrollment
Employees appointed for more than 120 days and 75% of full-time or greater are eligible for enrollment.
Effective Date of Coverage
- New employees have 30 days to either enroll for or waive coverage under this plan. Coverage is effective on the first day of the month after completion of one full calendar month of employment.
- Employees who do not enroll for coverage within the first 30 days but who later wish to pick up coverage must complete an evidence of insurability form and be approved for coverage.
