Annual Enrollment - Plan Year January 1, 2013 - December 31, 2013

 


The new plan year will run from January 1, 2013 - December 31, 2013.  Elections made during the annual enrollment period will become effective January 1, 2013.

Annual Enrollment for Medicare 65 plans will be October 1, 2012 - December 31, 2012.

Annual Enrollment will take place from October 1, 2012 through October 31, 2012.

FAQ

What do I have to do during Annual Enrollment?

  • If you contribute to one or both of the tax-saver flexible benefits plan spending accounts (medical or dependent) and you want to continue to participate in the next plan year, you must re-enroll. You will enroll online via PAWS (instructions provided in the Tax-Saver portion of the website below). If you do not re-enroll (which means selecting the amount you’d like withheld) you will not participate in 2013. Employees who do not have a PAWS account and wish to re-enroll will need to complete a hard copy form.
  • Otherwise, nothing. But there are a few things you SHOULD do.
    • You should check/update your beneficiary information for life insurance.
    • You should take an inventory of the benefits and insurance coverage you do have, making sure it is adequate or not too much.
What can I do during Annual Enrollment?
  • Switch between health insurance carriers (Active employees will do this via PAWS and instructions are in the Health Insurance section of this website below).
  • Make changes that are restricted by the Tax Saver plan, should you currently participate (add/drop dental/vision...).
  • You can choose to have your premiums tax sheltered for the coming plan year or you can choose to remove the tax-shelter on premiums (Tax Saver Premium Only plan)
  • You can participate in a Flexible Spending Plan for either Health or Dependent Care. (You MUST sign up for this each year and elect an amount for the account. This is not an automatic re-enroll.)
  • Increase your LSU Supplemental Life Insurance benefit amount (may increase by 1x annual salary in increments of $10,000 without underwriting approval) if you have not yet reached your maximum guaranteed amount.
  • Do nothing - if you don't want to make any changes to your insurance plans.

PLEASE NOTE: Employees who wish to add a dependent to their health plan OR who do not currently participate in one of the employee offered health insurance plans do not have enroll during annual enrollment. There are rate and benefit changes to the LSU First health plan. These are listed under the heading, "Health Insurance" below.

***Reminder of expanded coverage in several key areas that began July 1, 2011, as mandated by federal law (the Affordable Care Act and the Mental Health Parity and Addiction Equity Act):

  • The pre-existing condition (PEC exclusion will no longer apply to any employee or dependent under age 19.
  • Coverage will be offered for dependent children up to age 26, regardless of student, marital or tax status.  A covered child under age 26 who is or becomes incapable of self-sustaining employment is eligible to continue coverage as an overage dependent if OGB receives required medical documents verifying his or her incapacity before he or she reaches age 26.  The definition of incapacity has been broadened to include mental and physical incapacity. 
  • The individual lifetime maximum for benefits has been eliminated.  Annual dollar limits on essential benefits are also eliminated.
  • Preventive care (wellness) will be paid at 100% (no deductibles, co-payments, coinsurance) if services are provided by a network provider (LSU First does not mandate an in-network provider for wellness benefits).  Preventive care benefit limits no longer apply.

  • The plan member cost-share (deductible, coinsurance or co-payment) for inpatient and outpatient medical care and surgery will also apply to inpatient and outpatient mental health and substance abuse (MHSA) treatment.  There will no longer be a separate plan member cost-share for MHSA benefits.

 What Can I Do During Annual Enrollment?

  • Switch between health insurance carriers  (Health Plan Comparison and Summary)  Here are instructions on how to do that online:
    • Using your Internet browser software, such as Netscape or Internet Explorer, open the LSU home page at http://www.lsu.edu.
    • From the LSU home page, click on the PAWS link located on the navigational menu bar in the upper left-hand side of the page.
    • Once you reach the PAWS page (which can be accessed directly at http://paws.lsu.edu), click on the word "logon" located at the upper left-hand side of the page.
    • You will then be prompted to enter your PAWS logon ID and your password.
    • Once you have successfully logged in and are presented with your PAWS desktop, select the option Employee Resources, then select INS ANNUAL ENROLLMENT.
    • Insurance enrollment page will display your current demographic information and current insurance participation.
    • To elect a new insurance carrier - click on the plan of your choice, then click SUBMIT.
    • A confirmation page will appear with your desired election.  If the election is incorrect, press the back button and re-select your desired carrier.
    • If you experience difficulty, please call HRM (225.578.2258) 
  • *Remember* You can add a qualifying dependent to your health insurance at any time.  Ask HRM for appropriate paperwork.
  • Do nothing - if you don't want to make any changes in your health plan. 
  •  What is Changing with the Health Insurance Plans?

  • There is a rate change for some LSU First levels (Active Employees: Employee + Spouse and Family; Retirees: with No Medicare, Option 2) of coverage.  Click >>>HERE<<< for the rates.
  • Out-of-Pocket Maximums will increase for the LSU First plan on all levels as follows: The Co-insurance for Employee only will increase from $1,000 to $1,500;  Employee + Child(ren), Employee + Spouse will increase from $1,500 to $2,250; Family will increase from $2,000 to $3,000.

  • Prescription Drug Coverage for Brand Name and Specialty Drugs is amended. Members who utilize brand name and specialty drugs will pay a co-payment for each 30 day supply of the brand name drug and $120 for each 30 day supply of specialty drugs after the deductible has been met. Currently, after the deductible has been met, these drugs are being covered at 90% and the member is responsible for 10%. Generic drugs will continue to be covered at 100% once the member's HRA is exhausted. The co-payments will NOT go towards the out-of-pocket maximum.
  • SELECT Home Delivery for Maintenance Drugs with LSU First. Members who use maintenance drugs will be required to select whether or not to have your maintenance medications delivered through mail order/home delivery. Through mail delivery, a 90 day supply of the maintenance drug will be delivered to their home. Express Scripts will contact those members who have used maintenance drugs in the past 130 days in order to get the home delivery set up. You will be required to make a decision. Maintenance drugs may be refilled at a pharmacy twice before the decision is made. If Express Scripts is not contacted before your third refill, you will have to pay the full cost of your prescription until you contact Express Scripts with your decision. >>>HERE<<< is a list of the maintenance drugs.

  • Prior Authorization required for some medications. List >>>HERE<<<
  • Walgreens is now back IN-NETWORK for LSU First plan members. Express Scripts has reinstated their contract with Walgreens. Members who wish to have their prescriptions transferred to Walgreens should contact Walgreens directly.
  • PPO plan will now be administered by Blue Cross / Blue Shield and will have a nation-wide network.
  • HDHP with HSA plan will now be administered by Blue Cross / Blue Shield. It was formerly administered by United Healthcare.

REMINDER: Below are the changes that went into affect July 1, 2011.

  • Effective July 1, 2011, the age limit for dependent health coverage was extended on children up to age 26, regardless of student, marital or tax status.  
  • There is no pre-existing condition exclusion for individuals up to age 19.
  • Eligible preventive care benefits (wellness) paid at 100% if provided by a network provider for the PPO, HMOs and CDHP.  Eligible preventive care benefits under LSU First are also paid at 100% whether provider is in or out of the network.  (A list can be found on their website: www.lsufirst.org)
  • Lifetime maximums have been eliminated
  • Mental health and substance abuse treatment benefits have been enhanced to comply with the Federal Mental Health Parity Act.
  • Benefit added to PPO and HMO  health plans for over-the-counter proton pump inhibitor (PPI) medications with a prescription. (Proton pump inhibitors [or PPI's] are a group of drugs whose main action is a pronounced and long-lasting reduction of gastric acid production.)

  •  Health Plans Offered:

  • LSU First (Cigna) - A Consumer Driven Health Plan - offered in all regions to LSU System employees and retirees. (Note: there are two options to choose from: Option 1 and Option 2. The premium is lower under Option 2 but that option has a much higher deductible.) Click >>>HERE <<< for the LSU First Power Point presentation.
  • PPO - to be administered by Blue Cross / Blue Shield and offered in all regions statewide. 
  • HMO - administered by Blue Cross / Blue Shield of LA.
  • Medical HOME HMO - to be administered by Vantage Health Plan. This plan requires members to choose a primary care physician. The plan is offered in the Baton Rouge, Lake Charles, Monroe, Shreveport and Alexandria areas. *** Please note that there is now only ONE Vantage Medical HMO. For those that were in the former Medical HMO plans, if no election is made, their coverage will default to the consolidated version.***
  • CDHP with HSA - A High Deductible Health Plan with a health savings account (HSA) option, known as the Blue Cross Consumer Drive Health Plan (CDHP), administered by Blue Cross / Blue Shield. It features lower premium rates in exchange for a higher deductible. If the employee opens an HSA, the state will contribute $100 to jump-start his savings and also match his tax-free contributions made through payroll deductions up to an additional $400 per plan year. Unlike a health care flexible spending account (HCFSA) with a use-or-lose rule, the HSA does not require the employee to spend all annual contributions. Instead, the money remains in the HSA and grows—tax-free—from year to year. If the employee changes health plans or jobs, or retires, the HSA is his to keep. And from age 65 on, these HSA dollars can be used for any health care or non-health care expense with no penalty.
  •  LSU First Life Insurance

    If you are covered under the LSU First Health Insurance, the plan includes a $25,000 term life insurance policy and a $25,000 Accidental Death and Dismemberment benefit (AD&D), for employees and retirees only (not dependents). Please complete the beneficiary designation form only if you are enrolling in the LSU First Plan or you are currently in the LSU First Plan and did not previously complete this form or wish to change your beneficiary.  Submit directly to the AgCenter HR office.

    Information has been posted to the Office of Group Benefits website at www.groupbenefits.org under the annual enrollment section for each plan they have.  Here are the direct links: PPO, HMO, Medical Home HMO, and CDHP-HSA. 

    LSU First (LSU System Health Plan) members can use the 3 networks listed below which can be found via the LSU First website, www.lsufirst.org.  Click the "Search for Providers" link on the blue bar heading.

    For First Choice Providers, search by city. 
    For the Cigna Network, choose OPEN ACCESS PLUS as the network. 
    For the Verity, choose the Verity HealthNet Option.




What Can I Do During Annual Enrollment?

  • You can enroll in or cancel the Premium Only option of the Tax Saver Plan (use the Universal Enrollment Form in the Forms section below)
  • You can enroll in the Spending Account option (either or both - Dependent Care and Health Care).  This must be done each plan year.  Your election from last plan year will NOT carry over.  This will be done electronically on PAWS.  Instructions to do so follow:  If you do not have access to PAWS, please contact HRM.
    • Using your Internet browser software, such as Netscape or Internet Explorer, open the LSU home page at http://www.lsu.edu
    • From the LSU home page, click on the PAWS link located on the navigational menu bar in the upper left-hand side of the page.
    • Once you reach the PAWS page (which can be accessed directly at http://paws.lsu.edu), click on the word “logon” located at the upper left-hand side of the page.
    • You will then be prompted to enter your PAWS logon ID and your password.
    • Once you have successfully logged in and are presented with your PAWS desktop, select the option Employee Resources, then select FLEX ANNUAL ENROLL
    • Flex enrollment page will display your current demographic information and current flexible spending participation.
    • To elect a health care spending or dependent care spending account. Click: YES for desired account and enter the ANNUAL target amount. After entering the Target amount click: SUBMIT. 
    • A confirmation page and an email message will appear once the employee’s desired election has been submitted. If the election is incorrect, there will be a link in the email confirmation for the employee to return to PAWS to make changes. If you do not get a confirmation page, you are not enrolled.

**Note: Annual Target amounts will be rounded to even whole dollars in order to comply with IRS regulations and Payroll processing requirements.

What is Changing with the Tax-Saver Plans During Annual Enrollment?

  • Flexible Spending Account Maximum contribution will be capped at $2,500 (decreased from $4,000) for the 2013 plan year. $5,000 for the dependent care spending account will be in effect.   

General Information 

The Tax Saver Plan includes premium only and spending account options.

The premium only option allows you to tax shelter insurance premiums paid for coverage under the Group Benefits Program (the PPO, the HMO’S, the CDHP and the LSU First Health Plan), the vision plan and the dental plan. It does not shelter the Group Benefits dependent life premium. There is no charge for participating in this option. Employees who currently participate will be automatically continued January 1, 2013 unless they complete a new election.  Keep in mind that if you are participating in this plan, a decrease or cancellation of any coverage under this plan must be done during the months of October (after October 1), November or December and will have a January 1, 2013 effective date.

If your family has several hundred dollars worth of predictable medical expenses for which you will not be reimbursed through insurance plans (e.g. orthodontia, dental, eyeglasses, deductibles, co-payments, etc.) or if you have significant child care costs, the spending account portion of the Tax Saver Plan can probably save you money. Please refer to the Tax Saver page on our website for specific plan details.

Employees with medical or dependent care spending accounts must complete an election to re-enroll each year even if they are continuing the same deduction. This may be done on PAWS.  Instructions to do so are included in the Forms section of this website.  If you do not have access to PAWS, please contact HRM.




What Can I Do During Annual Enrollment?

What Can I Do During Annual Enrollment?

  • The Davis Vision Plan will continue to administer the vision benefit.  There are no changes to benefits or rates.

What Can I Do During Annual Enrollment?

  • Employees who currently participate in this plan may elect to increase their coverage by 1x their annual salary (or up to that in increments of $10,000) without evidence of insurability provided they have not reached the maximum of 5x their annual salary or $500,000 (whichever is less).  The Hartford asks that a new enrollment form be completed entirely for any updates or increases.  (A letter containing the form was mailed to all current participants.)  This can also be found in our Forms section below.
  • If you do not currently have this policy, you may apply for coverage at any time, but you will have fill out Evidence of Insurability paperwork and be approved to do so.  (See our site on Life Insurance for details.)

What is Changing During Annual Enrollment?

  • This plan is not making any changes to benefits or rates for this annual enrollment period.

General Information

All forms for coverage increase must be signed and dated between October 1 and October 31, 2012.  The effective date for the additional coverage elections will be January 1, 2013. 

Anyone wishing to apply for first-time coverage will need to get the forms to HR as soon as they can.  The effective date of coverage will depend on date of approval.
