Every year during the month of April employees are given the opportunity to make changes to their current health-related benefits (Health, Dental and Vision). Changes made during Annual Enrollment (April 1st - 30th) are effective July 1st.

Active and Retired Employee Health Insurance Rates - 2009-2010 Rates
Health Insurance
- Employees may enroll in health insurance coverage or add eligible dependents at ANY TIME (Annual Enrollment is a time for members currently enrolled to change plan carriers). The coverage effective date is determined by the date the form is received by our office. Applicants will need to complete the Group Benefits Enrollment / Change form, the Pre-existing Condition Form, and the Portability Form. Please contact the Agcenter HRM Office for more information.

Tax Saver Flexible Benefits Plan
- New Plan Administrator - Gilsbar Flex Spending Account Flyer Click here to link to their website
- Enrollment Form - Employees may enroll in the Tax Saver plan within 30 days of their date of hire, within 30 days of a qualifying event, or during Annual Enrollment. Members currently enrolled in a Spending Account must re-enroll each year. Members currently enrolled in the Premium Only Plan do not need to re-enroll. Refer to the Plan Document for more information.

- Direct Deposit Enrollment Form - Employees who enroll in a Spending Account must complete a new direct deposit form for reimbursement.

Ameritas Dental Insurance

The LSU AgCenter offers employees two dental plans, both are underwritten by Ameritas. Employees may enroll in one of the two plans within the first 30 days of employment or during Annual Enrollment.
Option 1 - Enhanced Plan Brochure and Enrollment Form - This form may be used by employees who are not currently enrolled in the Dental plan who would like to enroll.

Always Vision Insurance

The LSU AgCenter offers employees a Vision insurance plan underwritten by Always Vision. Employees may enroll in the Vision plan within the first 30 days of employment or during Annual Enrollment.

LSU System Life Insurance
- The LSU AgCenter offers employees a life insurance policy which is currently underwritten by ING. There will be changes to this plan which will be rolled out during the month of May. Details will be communicated once finalized.
