2008-2009 Annual Enrollment Information

Effective July 1, 2008 - June 30, 2009

Every year during the month of April employees are given the opportunity to make changes to their current health-related benefits (Health, Life, Dental and Vision). Changes made during Annual Enrollment (April 1st - 30th) are effective July 1st.

Meeting Information

  • Office of Group Benefits Annual Enrollment LSU Campus Meetings - Representatives from the Health Insurance plans and some of the Optional Insurance plans will discuss and answer questions about the insurance plans.
  • Human Resources Annual Enrollment Informational Meetings - Effective July 1, 2008, the LSU System Health Plan (formerly Definity) will be known as “LSU First.” The benefits will remain the same although there will be a few enhancements to the plan. The plan will be administered by Cigna. A representative from Cigna will discuss the plan as well as be available to answer questions about this plan. A representative from the AgCenter HRM Office will also be present to answer questions.
  • Statewide Meetings for Active and Retired Employees - Representatives from the Health Insurance plans will discuss and answer questions about the insurance plans (LSU System Health Plan representatives will only be present at the meetings hosted by campuses of the LSU System).
  • Statewide Meetings for Retirees with Medicare Part A and Part B - Representatives from the Health Insurance plans will discuss and answer questions about the insurance plans and coverage when secondary to Medicare (LSU System Health Plan representatives will only be present at the meetings hosted by campuses of the LSU System).

Health Insurance

  • Employees may enroll in health insurance coverage or add eligible dependents at any time (Annual Enrollment is a time for members currently enrolled to change plan carriers). The coverage effective date is determined by the date the form is received by our office. Applicants will need to complete the Group Benefits Enrollment / Change form, the Pre-existing Condition Form, and the Portability Form. Please contact the Agcenter HRM Office for more information.
  • Health Insurance Plan Rates - View the health insurance rates for active employees under the LSU First, EPO, HMO and PPO plans.

Please Note: The providers listed are subject to change. Please call your provider prior to service to validate network status.

Tax Saver Flexible Benefits Plan 

  • Enrollment Form - Employees may enroll in the Tax Saver plan within 30 days of their date of hire, within 30 days of a qualifying event, or during Annual Enrollment. Members currently enrolled in a Spending Account must re-enroll each year. Members currently enrolled in the Premium Only Plan do not need to re-enroll. Refer to the Plan Document for more information.
  • Tax Saver Plan Frequently Asked Questions (FAQ'S) - Review the FAQ's that other employees have asked in the past and learn how the answers may affect your individual situation.
  • Debit Card Application - Employees who elect to participate in the Spending Account(s) may elect to receive a Debit Card which may be used to pay for eligible expenses. Members who already have a Debit Card do not need to reapply for a new card unless they have lost their previously issued card.
  • Debit Card Instructions - Learn how the Debit Card works and determine if this option will be beneficial for you.
  • Debit Card Frequently Asked Questions (FAQ'S) - Review the FAQ's that other employees have asked in the past and learn how the answers may affect your individual situation and determine if the Debit Card will be beneficial for you.
  • Direct Deposit Enrollment Form - Employees enrolled in a Spending Account may elect to have their claim payments directly deposited into a checking or savings account. Members who do not elect to enroll in direct deposit will have their claim payments mailed directly to the home address on file in the Processworks system. Members currently enrolled in Processworks' Direct Deposit, do not need to re-enroll.

Ameritas Dental Insurance

Always Vision Insurance

  • The LSU AgCenter offers employees a Vision insurance plan underwritten by Always Vision. Employees may enroll in the Vision plan within the first 30 days of employment or during Annual Enrollment.
  • Plan Brochure and Enrollment Form - This form may be used by employees who are not currently enrolled in the Vision plan who would like to enroll.
  • Always Vision Change Form - This form may be used by members currently enrolled in the Vision Plan who would like to change their current coverage (Add or delete dependents or cancel coverage).

LSU System (ING) Life Insurance

  • The LSU AgCenter offers employees a Life Insurance plan underwritten by ING. Employees may enroll in this plan within the first 30 days of employment with guaranteed coverage or at anytime after with coverage subject to approval.
  • Enrollment Form - Members currently enrolled may elect to increase their coverage amount by $10,000 every annual enrollment (guaranteed issue) up to three times their annual salary, not to exceed $500,000.00.
  • Evidence of Insurability Form - Members who are not currently enrolled, may apply for coverage at any time by completing the enrollment form and the evidence of insurability form. After completion, please return both forms to the LSU AgCenter HRM office.
