Three life insurance plans are currently offered by the AgCenter.

LSU First (automatically comes with enrollment in the LSU First Health Plan - By HCC).
PRUDENTIAL (Group Benefits) - A group life insurance plan is offered through the Group Benefits Program. Half of the premium is paid by the state for the employee's coverage. The employee must pay the entire premium for dependent coverage. The employee may select from two options with a maximum coverage of $50,000.

THE HARTFORD TERM LIFE INSURANCE (LSU System) - This plan provides an option for group-term life coverage for eligible employees. It provides a death benefit equal to the coverage amount in effect at the time of death and payable to the named beneficiary. Coverage amounts available range from $10,000 to $1,000,000. Premiums are age-rated. Also available is accidental death and dismemberment coverage. Dependent life insurance can be purchased for the employee's spouse and children. Coverage for children may be added anytime without proving Evidence of Insurability.

Eligibility for Enrollment
Employees appointed for more than 120 days and 75% of full-time or greater are eligible for enrollment.
Effective Date of Coverage
- New employees have 30 days to either enroll for or waive coverage under this plan. Coverage is effective on the first day of the month after completion of one full calendar month of employment.
- Employees who do not enroll for coverage within the first 30 days but who later wish to pick up coverage must complete pages 2 - 5 of the Evidence of Insurability form, send the entire document to the HRM office for completion, and wait to be approved for coverage.
EXTENSION BENEVOLENT AND PROTECTIVE INSURANCE This is optional life insurance coverage offered to all Louisiana Cooperative Extension Service appointed employees. In addition to life insurance, it also provides for Accidental Death an Dismemberment (AD&D) coverage.
- The effective date of coverage begins on the first day of the month following the date of employment.
- The face amount of coverage reduces at age 65 and again at age 70 (or upon retirement at any age).
- The AD&D portion of the coverage cancels at age 70 or upon retirement.

LSU FIRST LIFE INSURANCE For those employees enrolled in the LSU First health plan, option 1 or option 2, there is an automatic enrollment into the $25,000 life and $25,000 AD&D policy with HCC. This enrollment comes at no extra cost and is for the employee only. The policy does not cover spouse or children. If currently enrolled in LSU First, or planning to enroll, please complete the Beneficiary Designation form and the original to the HRM office. Please keep a copy for your own records.
